The R.N. Case Manager is an integral part of the hospice interdisciplinary care team and, as such, is responsible for coordinating the total care of the hospice patient and family.  This position reports to the Patient Care Coordinator and supervises L.P.N.s and C.N.A.s assigned to their patients.  The Case Manager will work closely with volunteers and be a liaison with community referrals, healthcare professionals and families to provide a greater understanding of hospice care and philosophy.
